A 6-week physician led program to gather information to inform your midlife health goals

Meeting 1
We will discuss your health, your concerns, goals and preferences to craft a testing plan most relevant to you.
Some of the tests may be covered by insurance, but many may not be; we will discuss cost and establish the plan for which tests to pursue.
You’ll leave this meeting with a list of the tests, prices, and scheduling.

Meeting 2
We will meet to review and discuss the findings of the test results that have been completed in the first 3 weeks.
We will discuss and apply the findings to your health and healthcare goals for the year ahead.
You’ll leave this meeting with an interim consult note and copies of test results for your files.

Meeting 3
We will meet to review and discuss the findings of the test results that have been completed over the 6 weeks.
We will discuss and apply the findings to your health and healthcare goals for the year ahead.
You’ll complete the Midlife Health Intensive with a finalized consult note and copies of all test results.

$3,500 payable to Amy Boutwell MD
Testing is paid separately to test providers
FSA/HSA eligible
